- Change of address and/or phone number
- Coverage change for you and/or your eligible dependents.
We are also here from 8:30 to 5:00 Monday through Friday to assist you with benefit and claim questions.
It is important to monitor your mail as member materials and other documents are sent out by the Health Fund and our business associates, including Alicare, Alicare Medical Management (AMM), BeneCard, and Sele-Dent.
